The United Ostomy Association of Canada Inc, in accordance with applicable privacy laws, is committed to protecting the privacy of its members, volunteers and officers and any other persons about or from whom the United Ostomy Association of Canada Inc collects information.
The United Ostomy Association of Canada Inc embraces the principles of the Canadian Standards Association Model Code for the Protection of Personal Information to ensure that all personal information is properly collected, used only for the purpose for which it is collected and is disposed of in a safe and timely manner when no longer required.
The United Ostomy Association of Canada Inc. is responsible for maintaining and protecting the personal information under its control. The United Ostomy Association of Canada Inc. has a Privacy Officer who is responsible to ensure that the Association complies with its privacy obligations in accordance with applicable laws.
The United Ostomy Association of Canada Inc. may, from time to time compile and use data in an anonymous, aggregate form to generate statistical data for planning and research purposes, and to meet reporting requirements of various sources.
The United Ostomy Association of Canada Inc. also collects and uses personal information for a variety of other purposes including, but not limited to:

The United Ostomy Association of Canada Inc shall identify and explain the purposes for which it collects personal information to the person from whom the personal information is being collected, before or at any time the information is collected.

Occasionally the person’s consent may be implied by the person’s conduct with the United Ostomy Association of Canada Inc. When it is appropriate, written consent will be obtained.
Written consent will be kept on file for as long as the information is reasonably necessary. A person may withdraw his /her consent at any time, subject to legal or contractual restrictions and reasonable notice. The person will be notified of any implications of the withdrawal.
Information collected will be limited to that required for the purposes identified by the United Ostomy Association of Canada Inc. The United Ostomy Association of Canada Inc. is committed to collecting information in a fair, open and lawful manner.
Limiting Use, Disclosure and Retention
Personal information shall not be used or disclosed for purposes other than for which it was collected, except with the consent of the person or as required by law. All collected personal information shall be destroyed, erased or made anonymous as soon as the purpose for which it was collected in no longer relevant, or permitted by law.

Personal information will be protected by security safeguards that are appropriate and of a type required by the sensitivity of personal information.

Access to Personal Information
Access to personal information is restricted to those UOAC volunteers who need to know in the course of their duties. Access to personal information will also be granted, where the United Ostomy Association of Canada Inc. is legally required to release the information and provided that disclosure does not violate any applicable statutes to the person to whom the information pertains where there is an appropriate written request. The use and disclosure of personal information will be granted within a reasonable amount of time.
Challenging Compliance, Complaints or inquires about the collection, use, disclosure or retention of personal information and the United Ostomy Association of Canada Inc. compliance with these set principles should be directed to the Privacy Officer. This Privacy Office will investigate complaints and ensure appropriate measures are taken.
